2010 Award Nomination
AANAC takes great pride in the achievements of all of our members. Some individuals, however, go above and beyond the call of duty in their daily work, touching the lives of residents in special ways, making their load just a little bit lighter. We would like to honor:
- A Nurse Assessment Coordinator
- An Administrator who supports clinicians involved in the MDS process.
- The AANAC member who has contributed most to AANAC.
- A Director of Nursing.
With so many talented and noteworthy people, we would like your help in submitting nominations for a facility based MDS Coordinator, an Administrator, a Director of Nursing, and an AANAC member who goes that extra mile.
The nomination deadline is February 10, 2010.
HOW TO SUBMIT A NOMINATION
Individuals, organizations, or groups may submit nominations. Please use a separate form for each nomination. All awards will be presented at the AANAC Spring Conference to be held in Las Vegas, April 29-30, 2010. Winners will be notified by February 26, 2010, but will not be publicly announced until the Awards Luncheon at the conference on Friday, April 30, 2010.
Each Nomination should include:
- Completed nomination form to include a chronology of the individual’s involvement in the MDS process.
- Cover letter explaining why you believe the nominee should be considered for this award.
- A digital picture 300 dpi saved as a jpeg or .eps
Each Nomination may include:
- Up to three letters of recommendation supporting the nomination; and
- Other materials supporting the nomination.
These nominations must be submitted electronically to
info@aanac.org or faxed to 303.758.3588.
The Awards Committee is comprised of AANAC Members. The Committee will review the nominations and select the winners.
