Question: If a resident is in a locked unit related to dementia, and they are free and safe to wander around, do you still code wandering (E0900) on the MDS? If it is already on the care plan, do we still code it?

Answer: Yes. If a resident wanders on the locked unit, wandering should be coded. You code wandering even if it has been care planned. E0900 records the presence of wandering during the look-back period.
